ICYMI: DQA’s Guidance on Background Check Renewals and Login for MRS

4-Year Renewals of Background Checks for Entity Operators & Non-Client Residents
Background checks must be periodically renewed for persons approved by the Department of Health Services to operate entities (e.g., licensees, owners, legal representatives, etc.) or who live in entity facilities as non-client residents. Please read DQA Memo 25-001, which is intended to provide information to entities about:
- How often background checks must be renewed
- How to request a background check from DQA
- How to determine your required date of renewal
New login for the Misconduct Reporting System
MyWisconsin ID is the State of Wisconsin’s identity solution that allows citizens the ability to securely access participating online state services and systems using a single user ID and password. Beginning March 19, 2025, MyWisconsin ID will be needed to report through the DQA Misconduct Reporting System. Please request reporting access for your entity at your earliest convenience to avoid reporting delays. Requests are processed within two business days.
